- Evidence of aviation insurance Evidence of worker’s compensation
- Evidence of general liability
- Evidence of automobile insurance
- Sponsor letter from PSU unit
- Film agreement (PSU will initiate if photos or video) indemnification agreement it not capturing footage
Attachments for ALL (PSU & 3rd Parties)
- Copy of Part 107 cert (attach in Pilot info)
- Copy of FAA UA registration card (attach in Aircraft info)
- Site authorization (if not PSU property; if on PSU property, UA office will request authorization)

- Students flying as part of paid work responsibilities = verification needed from department
